Questionnaire Wills and Powers of Attorney

CONGRATULATIONS! YOU HAVE DECIDED TO PROTECT THE FUTURE INTEREST OF YOUR LOVED ONES. This form is easy to understand and should take you a few minutes to complete. It provides the essential information and building blocks for the preparation of your basic Will and Powers of Attorney.

If you have a blended family or own a business, then a basic Will may not be suitable for you. In such case, let us know and we will have a lawyer contact you to discuss your circumstances. An additional cost may be applicable due to the complexity of your circumstances.

YOU AND YOUR SPOUSE, IF APPLICABLE, MUST EACH COMPLETE AND SUBMIT THIS FORM. Please provide your complete home mailing address and telephone number where you are most likely to be reached during the day and evening. Please use full legal names throughout this questionnaire.

Upon submitting the completed form below, you will be contacted by our office to review and approve the draft Will and Powers of Attorney, and to schedule an appointment to sign the same.   Payment of our account will be required at your scheduled appointment.
